

Southeast Seniors is a non-profit, neighborhood based organization of health care professionals and neighbors helping people 65 and older remain independent and safe at home.

Our service area includes the neighborhoods of Como, Marcy Holmes and Prospect Park in Southeast Minneapolis, defined by zip code 55414.
Our Vision
Seniors at home: comfortable, connected, empowered, safe.
We provide Nursing Services: care after hospitalization, care in chronic illness, management of medications, care of feet and nails; Ask a Nurse/Blood Pressure Clinics; and Home Health Aide Services. Our nursing and home health aide services are offered through a contract with Fairview Home Care and Hospice. We work with Medicare and all insurance companies in providing these services. We also have a sliding fee scale.
Other services include: visiting volunteers; connecting seniors with services such as Lifeline, Vision and Hearing Loss Resources, Meals on Wheels, chore services and transportation. We also sponsor inter-generational activities and health seminars in the community. There are no charges for volunteer or service coordination services.
