Service Area

Individual Support Services

Southeast Seniors helps older adults and their caregivers live at home as long as possible with these services:
  • In-Home Visits
    In-home visits by our outreach nurse and social worker include a free home assessment and consultation to help you live well at home. Also, follow up wellness checks, health counseling, care in chronic illness and advocacy with health professionals. 
  • In-Home Nursing Care
    In-home nursing care, available on a sliding fee scale through SES, provides care coordination after hospitalization, foot care, and medication management through Fairview Home Care.
  • Caregiver Support
    Caregiver support to help you care for your loved one at home.
  • Rides
    Rides to medical appointments and grocery store provided by neighborhood volunteers.
  • Friendly Visits
    Friendly visits by volunteers provide companionship and caregiver respite.
  • Caring Calls
    Caring Calls connects seniors with weekly phone calls from a volunteer for a friendly chat and check-in.
  • Home and Yard
    Home and yard chore help by neighborhood volunteers. 
  • Connection to Services and Resources
    Connection to local services and resources, assistance in completing forms and applications and personalized problem solving and guidance to get your needs met.  
Fees
There are no charges for volunteer services and outreach nursing services. Costs for nursing services are kept as low as possible.

Medicare, Medicaid, private insurance (e.g. Blue Cross/Blue Shield, U Care, Health Partners, Medica, VA) and other third party payers are billed, whenever appropriate, for nurse and home health aide/homemaker services. Remaining costs are adjusted on a sliding fee scale.

The nurse will determine the exact cost for services on the first visit, which is free.

​Staying in your home can sometimes be a matter of finding practical solutions.  Learn about adaptive equipment, smart home technology, and more that can help you with the activities of daily living, maintaining your connections with others, and living well at home.

Sue Redepenning is the owner of LiveLife Therapy Solutions and the Program Director for Technology for HOME, a state-funded program which provides assistive technology (AT) for people with disabilities throughout Minnesota. Sue leads a team of twelve AT consultants (Occupational Therapy, Speech-Language Pathology, Physical Therapy, Nursing and Engineering). Sue is an Occupational Therapist and has worked in the field of AT for over 20 years. She has presented at AT-related conferences locally and nationally.

Pen Pal Program

Meet your PenPal at the Final Party!
Like to get mail that is not a bill?  Like to write letters and make a new friend in the process? 

People 65 and over in the 55414 zip code are invited to join our Pen Pal Program.  Letters are exchanged with 4th graders at Marcy School once a month. 

Participate in the comfort of your own home.  You don't have go anywhere to correspond with a young person.

How Does it Work?
​Letters are exchanged with a 4th grade student at Marcy School once a month or more if each partner is so inspired.  Students send the first letter in the fall to introduce themselves and start the process.

Southeast Seniors can provide you with stationary, envelopes and stamps if needed - just let us know!
Wider Scope
Students learn to write a neat and legible letter telling their story and sharing their lives. Some have grandparents in their lives, and some are grateful to have an older someone to be important to them.  Being a Pen Pal helps students learn to express themselves on paper.

Why Get Involved?
​Connecting with a UMN student can open doors for both you and the student. Learn what life is like for a student now and help them connect with their neighborhood. Students want to see how their studies can be applied to life around them.  Your involvement can open doors for them as they can benefit from your experience and knowledge.  You gain a new friend and a new perspective on student life.
Who Can be Involved?
​Anyone 65 and older can be involved! Students often have 20-30 hours during a semester to give (September - December or January - May). They are willing to help around the house and/or yard, play games, cook with you, and lots more. Set up a time to meet that works with your schedule.
How to Get Involved?
​Students begin classes in mid-January and want to get connected very soon afterwards. Complete the short form below and we'll make sure you meet someone as soon as school begins.

Blood Pressure Check / Ask A Nurse & Service Coordinator

Residents of Holmes Park Village and Labor Retreat Apartments are invited to:
  • Meet with our Outreach Nurse for a blood pressure check and discuss health and nutrition issues.
  • Talk with our Service Coordinator who can provide information and assistance finding resources and help filling out paperwork. 
  • Get support for living at home.

Lunch Bunch Topics

March 17th: Why Friendship Matters in a time of COVID-19 

This interactive presentation explores the value and importance of friendship in these turbulent pandemic times and looks at why finding creative ways to engage with others is so important to our well-being. Staying connected in the face of social distancing matters. Nourishing meaningful relationships and investing in new ones provides a powerful antidote to loneliness. Come connect with us. Together, we can harness the power of friendship and be there for each other when it matters most.

Georgia Afton takes her professional work in elder services personally. She originally with worked for her current employer – Little Brothers – Friends of the Elderly (LBFE) – from 1999 to 2003 as the organization’s development and communications director. To this day, she cherishes the memory of several Elder Friends made during those years who added joy, meaning and wisdom to her life. Those friendships, coupled with navigating her own parents’ elder years from overseas, drew her back to LBFE in 2015. She now serves as the organization’s community outreach director and is happy to, once again, embrace the work of ending elder loneliness and isolation through the power of friendship. Georgia’s previous nonprofit work has included communications and development roles with Nonviolent Peaceforce and Clare Housing. She earned a B.A. from University of Minnesota and an M.F.A. from Hamline University.

Library Book Delivery & Virtual Book Club

In partnership with the Arvonne Fraser library, Southeast Seniors offers no-contact home delivery of library books for  people over the age of 65 who reside in our service area. There is no cost to receive book delivery! Please call Southeast Seniors at 612-331-2302 to sign up. Staff will help complete a short form including information about the genres of books you enjoy so that the librarians can choose titles that align with your interests. Southeast Seniors & librarians from the Arvonne Fraser library are partnering to offer a monthly book club, which meets virtually via Zoom. The librarians provide copies of the selected book to Southeast Seniors, and we arrange home delivery for participants who'd like to borrow a copy of the book. Please call us at 612-331-2302 or email us at info@seseniors.org to sign up! These is no cost to participate, and you can attend once or participate monthly.
